We understand that Children and Young People enter the Care System filled with trauma, rejection and fear, and are often seen as high risk because they have complex behaviour, social and emotional difficulties, that stem from their journey into Care and through Care, which has an impact on their mental health and emotional wellbeing and on their outcomes and life chances.

To help improve their Care Experience, outcomes and life chances, we guide our Mentees through our Intervention and Support Mentoring Programmes designed to change the way they think about themselves, and empower them with all the essential Skills they need to Believe and See that they do have the ability to achieve and have the life and future they hope for, along with amplifying and bringing an understanding of their True Voice at relevant Children’s Social Care Meetings, as they journey through the Care System. Access to Specialist Therapeutic Support can be included in this Programme for each Mentee.

Our Programmes are delivered by The Transformed You Care Experienced ‘Intervention and Support Mentors’ who also adopt the role of:

  • Advocate to elevate and amplify the ‘True Voice’ of the Child and Young Person in Care and Care Leaver at meetings with Social Care and Professionals, to help them see and understand the Care Experience Journey from the Child or Young Persons’ perspective; along with empowering the Child and Young Person in Care and Care Leaver to express their ‘True Voice’

  • Adviser and Coach using their expertise to support the Child and Young Person in Care toward achieving their personal goals.

All Mentors are DBS checked.

Our Programmes are delivered throughout the Academic School Year (Term Time Only).

However, where it is deemed that our Mentees on our 1:1 Programme, are in need of support over the longer periods of not being in education, in particular the Summer Holiday, our service will be provided to work with them in readiness for the new Autumn School Term.

1:1 Intervention & Support Mentoring Programme

Delivered in line with the following flexible options:

  • Option 1 Mentee to only embark on Phase One and Phase Three of the Intervention and Support Mentoring Programme.

  • Option 2 Mentee to embark on the Full Three Phased Intervention and Support Mentoring Programme.

  • Children and Young People in Care

  • Year 3 – Year 11

  • To take place at The Transformed You Hub or at the Mentees’ School

  • For a minimum term of 20 months

Phase One

Our Mentors will plan and facilitate personalised 1:1 Intervention and Support Mentoring Programmes for each Mentee, to guide them through a transformative process to change the way they think about themselves, and empower them with all the essential Skills they need for them to Believe and See that they have the ability to achieve and have the life and future they hope for. We also hear and amplify their True Voice at relevant Children’s Social Care Meetings, as they journey through the Care System.

Our Mentor will issue Weekly Reports to the Designated Team of Professionals (Virtual School, Education Provision, Social Care – Mentees Social Worker), detailing each session as the ‘True Voice’ of the Mentee, along with providing suggestions of positive recommendations to support the Mentees progress.

The Mentor will also be able to participate in Child In Care Reviews and PEPs, with and on behalf of the Mentee.

Please note: Due to the sensitive nature and confidentiality of the issues shared by the Mentee in the sessions, reports are not sent to anyone outside of the Designated Team of Professionals, as in doing so this may have a negative impact on the progress of the Mentee along with impacting on the trust they will begin to have in The Transformed You process.

Recommended Time Frame of Sessions in the following Stages:

  • Stage 1 We begin with two school terms tailored to address the identified issues of the Mentee, with the 1:1 sessions being delivered at two per week for 1hr per session. An agreed date will be set to review progress at a midway point of the two school terms, with the designated Team of Professionals (Virtual School, Education Provision, Social Care). Subject to the progress and engagement of the Mentee to the Programme, the sessions will continue at two per week for 1hr per session with a review taking place at the end of the two school terms in order to move on to Stage 2.

  • Stage 2 If the Mentee has begun to make positive steps forward, the sessions will then reduce to one per week for one school term with an agreed date set to review progress in order to move on to Stage 3.

  • Stage 3 If the Mentee continues to make positive steps forward the sessions will further reduce to fortnightly for one school term with an agreed date set to review progress in order to move on to Stage 4.

  • Stage 4 If the Mentee continues to make positive steps forward, the sessions will further reduce to one per month for an additional school term with an agreed date set for the Mentee to transition on to Phase Three, our Letterbox Contact or In Person Drop In Support at The Transformed You Hub.

Phase Two

Transitioning on to Phase Two, our After Programme Support, the Mentee will continue to receive tailored support from their Mentor, on a Monthly basis at 1½ Hour Sessions.

The Mentor will continue to work in partnership with Children’s Social Care and relevant Professionals.

Monthly Reports will be issued by our Mentor to the Designated Team of Professionals (Virtual School, Education Provision, Children’s Services – Mentees Social Worker/Young Person Advisor) detailing each session as the ‘True Voice’ of the Mentee, along with providing suggestions of positive recommendations to support the Mentees progress.

The Mentor will also be able to participate in Child In Care Reviews and PEPs with and on behalf of the Mentee.

Phase Two After Programme Support will be in place as follows:

  • Support Mentoring sessions for 12 months

  • Monthly detailed Report to be sent to the relevant Core Professional Team of Support, be that the Virtual School, Allocated Social Worker, Designated Teacher and Young Persons Advisor

  • The Mentee will transition onto Phase Three Letterbox Contact or In Person Drop In Support with their Mentor at The Transformed You Hub, at the end of the 12 months

Phase Three

Where the Mentee has developed a positive professional relationship with their Intervention and Support Mentor, they will have the opportunity to remain in contact through our Letterbox Contact by way of:

  • Email

  • Phone

  • In Person Drop In Support at The Transformed You Hub

This is Phase Three of our Intervention and Support Mentoring Programme.

Please Note: Any concerns raised by the Young Person will be emailed to the current Allocated Social Worker, Young Persons Advisor, Virtual School Lead, with Safeguarding Issues passed on to the Local Safeguarding Board.
