What is The Transformed You Referral Process for the  1:1 Intervention and Support Mentoring Programme?

Referrals to book our 1:1 Intervention and Support Mentoring Programme must be made by the commissioning Local Authority Children’s Services Departments and/or Local Authority Virtual School.

The Referrer will need to complete our website Contact Form. Once received we’ll send you a Referral Form to complete.

On the Referral Form we’ll ask you to provide detailed information about the Child or Young Person In Care or Care Leaver you believe will benefit from The Transformed You 1:1 Intervention and Support Mentoring Programme.

Once completed and returned The Transformed You will make a decision as to whether our Programme is suitable for the Child or Young Person In Care or Care Leaver and inform the Referrer.

If suitable, The Transformed You will arrange with the Referrer an agreed date and time to complete an Initial Assessment and Introduction with the Child or Young Person in Care or Care Leaver.

During the Initial Assessment and Introduction, if the Child or Young Person in Care or Care Leaver states that they are willing to engage in our 1:1 Intervention and Support Mentoring Programme, we will proceed to provide you with our Service Contract, Terms of Business and Invoice for 50% of the Full Programme Costs in relation to the agreed Option 1 or Option 2  for the Three Phase + Initial Assessment and Introduction Costs.

If the Child or Young Person in Care or Care Leaver is unwilling to engage, we will send you an Invoice for the Initial Assessment and Introduction.

If the Child or Young Person in Care or Care Leavers agrees to engage, will the Programme commence directly after the Initial Assessment and Introduction?

The Programme will commence after completion of The Transformed You Service Contract, and when the Invoiced 50% of the chosen Programme Costs + Initial Assessment and Introduction Costs paid and cleared into The Transformed You Account. The 50% received is an indication of your commitment to using our services and is non-refundable.

When will the remaining 50% need to be paid?

One Month after the commencement of the 1:1 Intervention and Support Mentoring Programme, the Client shall pay the remaining balance in monthly instalments to The Transformed You in line with the terms of the Invoices issued.

In what geographical areas do you deliver the 1:1 Intervention and Support Mentoring Programme?

The Transformed You are currently able to deliver our 1:1 Intervention and Support Mentoring Programmes in Barking & Dagenham, Tower Hamlets, Havering, Newham, Redbridge, Hackney and Waltham Forest, with reasonable distance and travel time considered.

The 1:1 Intervention and Support Mentoring Programme can also take place at The Transformed You Hub.

We do not deliver this Programme via any Online Virtual Platforms.

What happens if the Child or Young Person In Care or Care Leaver is relocated?

If the Child or Young Person in Care or Care Leaver is to be relocated to an alternative Foster Care Placement, Residential Children’s Home or Education Provision, within the East London and Essex region, The Transformed You will need to be informed as soon as is practically possible in order for a smooth and continuous delivery of the 1:1 Intervention and Support Mentoring Programme to be maintained.

If the Child or Young Person in Care or Care Leaver is to be relocated outside of the East London and Essex region, The Transformed You will need to be informed as soon as is practically possible in order for us to modify the remaining Programme Sessions or sensitively transition our Mentee on to Letterbox Contact.

Is this the same process for Long Term Service Contracts?

Group/Peer Mentoring Programmes

What is The Transformed You Referral Process for the Group/Peer Mentoring Programmes?

Referrals to book our Group/Peer Mentoring Programmes (My Life, My Skills and My Culture, My Identity, My Heritage) must be made by the commissioning Local Authority Children’s Services Departments and/or Local Authority Virtual School.

The Referrer will need to complete our website Contact Form. Once received we’ll send you our Referral Service Contract to complete, along with our Terms of Business, Invoice for the Full Programme Costs and the Group/Peer Mentoring Programmes Wave details allocated to the Child or Young Person In Care or Care Leaver.

On the Referral Service Contract we’ll ask you to provide detailed information about the Child or Young Person In Care or Care Leaver you believe will benefit from The Transformed You Group/Peer Mentoring Programmes.

What Geographical Areas Do You Cover?

Our Programmes and Our Pantry are available to Barking & Dagenham, Tower Hamlets, Havering, Newham, Redbridge, Hackney and Waltham Forest.

What are your Safeguarding Procedures?

In order to Safeguard our Mentees, The Transformed You will follow our inhouse Safeguarding Policy and adhere to the Safeguarding Policy and Procedures of the named Education Provision and the named Local Authority.

All Transformed You Mentors have an up to date DBS Certificate.

Once received we’ll contact you to discuss the details of the Booking and agree a date for delivery.

We’ll then provide you with our Service Contract, Terms of Business and our Invoice.
